Close Teammates…Very close

Which two teammates suffered mechanical failure on the same lap in two separate Grands Prix?  What team was it?  What races were they?  What laps? And what year was it?  As a hint:  The first race saw and engine failure and a turbo failure while the second race saw two turbo failures.  That should give you the approximate year.
